ScienceDaily (Nov. 29, 2012) ? Using standard tools of the molecular-biology trade and a new, much-improved animal model of a prevalent but poorly understood tropical parasitic disease called urogenital schistosomiasis, Stanford University School of Medicine researchers were able to obtain "snapshots" of shifting gene activity levels during the early, acute phase of what for most becomes a chronic bladder infection.
The findings, described in a study to be published online Nov. 29 in PLoS Neglected Tropical Diseases, could lead to new diagnostic and therapeutic approaches to urogenital schistosomiasis, a chronic disease that infects about 112 million people, almost entirely in developing countries and particularly in Africa, said Michael Hsieh, MD, PhD, assistant professor of urology and the study's senior author.
"Schistosomiasis is a huge global health problem and one of the most neglected of the neglected tropical diseases," said Hsieh. "Some 150,000 people die each year from urogenital-schistosomiasis-induced kidney failure. And this doesn't include deaths from bladder cancer or as a result of increased susceptibility to other infections, such as HIV, attributable to schistosomiasis-generated genital tract damage." Estimates that take into account chronic schistosomiasis' overall impact on quality of life place it on a par with other world-class health scourges such as malaria and tuberculosis.
Schistosomiasis is caused by a parasitic worm of the genus Schistosoma. Different Schistosoma species infect different organs, said Hsieh, whose research focuses on infection-induced bladder inflammation. S. haematobium, which is responsible for infection of the urogenital tract, is acquired by exposure to contaminated water. "The parasite larvae can smell fatty acids in our skin as well as detect motion. They're like smart bombs, equipped to find us."
After invading the skin and a pass through the circulatory system, the larvae mature to adulthood, lodge in blood vessels of the bladder wall and other pelvic organs and begin to lay eggs. A single worm can extend to several millimeters, lay hundreds of sand-grain-sized eggs every day and live for years to decades, Hsieh said.
In a study published recently in the British Medical Journal, John Ioannidis, MD, DSc, a Stanford professor of medicine, deplored the dearth of treatments for neglected tropical diseases including schistosomiasis. There are no vaccines for preventing it, and only a single drug, praziquantel, is approved by the World Health Organization for treating it. While cheap and relatively effective, it doesn't cure all infections. Plus it tastes terrible, which causes a surprisingly large number of people to stop taking it. Experts believe it's just a matter of time before the parasite develops resistance to praziquantel.
Despite a pressing need to come up with good anti-S. haematobium agents, almost all the existing medical literature is on other Schistosoma species. That's because until recently there were no practical animal models for S. haematobium infection. "We know next to nothing about the earliest molecular events in the bladder after eggs enter the tissue," said Hsieh. "When you try to naturally infect mice by simply exposing them to the parasite, the mice develop an infection in their liver or intestine instead of their bladder wall."
In March of this year, Hsieh and his colleagues reported, in the journal PLoS Pathogens, that they had solved this problem by injecting S. haematobium eggs directly into the bladder wall of ordinary laboratory mice. For the new PLoS-NTD study, Hsieh and his colleagues used this mouse model to synchronize the initiation of bladder infection by the parasite, thereby synchronizing the timing of post-infection changes that take place in bladder tissue so the researchers could better observe the parasitic infection's early trajectory. They injected about 3,000 parasite eggs into each of a few dozen lab mice and used microarray technology -- a method of globally measuring the activity of virtually every gene in a tissue -- to monitor thousands of genes' activity levels in bladder-wall tissue over the ensuing weeks.
The investigators set their microarray probe's sensitivity to flag any gene whose activation level either increased or decreased by a factor of two or more. They monitored thousands of genes whose activity levels were either amped up or tamped down at one, three and five weeks post-infection. These changes peaked at merely three weeks post-infection. "This is an important result," said Hsieh. "It shows that continuous waves of egg deposition, not just the initial schistosome infection, are what's generating chronic disease. So if you can kill the worms flat-out, you should be able to halt the cycle."
It would have been impossible to see this simply by observing patients, said Hsieh. "With the natural human infection, you never know when any given person is initially infected -- unless it's that unfortunate American tourist that got infected while swimming in Lake Victoria on a weeklong Africa visit," he said.
The team also noted that these gene-activity changes fell into clusters. "Very soon after infection, we saw changes in activation levels in a cluster of cancer-associated genes. Likewise with another cluster of genes involved in fibrosis, or scarring, in the bladder. This confirms that fibrosis kicks in very quickly," said Hsieh -- a not unexpected result, as parasite eggs' burrowing causes significant bladder-wall damage.
On the other hand, within the first week there was a general suppression of genes associated with maintaining the integrity of the bladder wall. Inadequate production of this group of molecules would be expected to result in a leaky bladder wall, said Hsieh, and it probably did. "This happened at a time when the mice were shedding eggs in their urine. So it probably accounts for not only a lot of urinary symptoms but a significant aspect of the disease's transmission -- the expulsion of eggs into the urine, which eventually contaminates another body of water," he said.
Finally, as has been observed in infections produced by other Schistosomal species, the Stanford team noted signs of a shift in character of the immune response toward a state that it is oriented to tissue repair and to isolating pathogens -- effectively attempting to enclose them in cement-like jails called granulomas -- in order to prevent further tissue damage. (This mode of immune response may be best-suited to combating relatively large pathogens that would be likely to withstand an attempt by the immune system to kill them outright in a frenzy of inflammatory fury.)
Hsieh's group corroborated their microarray results at both intracellular and macroscopic levels with ultrasound, microscopic and molecular assessments. For instance, discernible evidence of scarring in bladder-wall tissue coincided with microarray readings indicating fibrosis-gene activation.
"Schistosomes have evolved with mammals for millions of years," said Hsieh. "Our shared evolutionary history makes our findings in mice likely to be relevant to humans. These observations may also have implications for fibrotic disease in general. Understanding fibrosis is very important, because it is involved in cardiovascular disease, cancer and other diseases accounting for up to half of all deaths in the developed world."
The S. haematobium mouse model may also pinpoint biomarkers that enhance the detection and monitoring of incipient bladder cancer, a poorly understood disease.

The sheer number of efforts aimed at improving the quality and efficiency of the U.S. health care system -- ranging from portions of the national Affordable Care Act to local programs at individual hospitals and practices -- reflects the urgency and importance of the task. One aspect that has received inadequate attention, according to three physicians writing in the January 2013 issue of Academic Medicine, is training the next generation of experts needed to help lead these efforts. In their Perspective article, which has been released online, the authors propose a framework for career development in what they call "health systems improvement," a term that encompasses a broad range of activities -- including management, research and public policy -- to improve the quality and efficiency of our systems of care.
"My co-authors and I each had experience in nonclinical fields such as government, consulting or law before entering medical school, leading to countless conversations with our classmates and co-residents about atypical career paths," explains Clay Ackerly, MD, MSc, management and policy fellow at Massachusetts General Hospital and the Massachusetts General Physicians Organization, a co-author of the Academic Medicine article. "During these conversations most trainees echoed common themes of wanting to have careers that allow them to improve the quality of our care delivery systems. However, most of them also had difficulty articulating their exact interests, and it became clear that we lacked a common lexicon to discuss their specific interests and potential career opportunities."
The authors -- also including Ami Parekh, MD, JD, medical director of Health Systems and Innovation at the University of California, San Francisco, and Daniel Stein, MD, senior medical resident at Brigham and Woman's Hospital -- write that while a few academic medical centers have developed programs addressing health systems improvement, many medical trainees "may become overwhelmed by the sheer number of, and lack of clarity among, possible career paths." Since many of today's health care leaders reached their current positions through what the authors call "circuitous and often serendipitous career paths," their ability to guide and advise young physicians may be limited.
Based on a series of conversations with medical students, residents and leaders in health systems improvement, the authors put together their framework. It starts with three core focus areas -- research, policy and management -- defines three intersections -- policy advising, policy translation, and implementation science -- and includes examples of potential careers, such as academic administration, health services research and government relations.
"The framework is largely a collection of definitions -- a new taxonomy, if you will -- and while some may disagree with the dividing lines, we wanted to put some structure around the passion so many trainees feel for improving the care of patients," Ackerly explains. "We hope this framework can help anyone with that passion -- from any background, not just MDs -- find productive ways to develop their interest and have an impact."
Ackerly and his co-authors suggest that trainees interested in careers in health systems improvement take a few simple steps to help guide their early career development. Some are as basic as reflecting on their own preferences, including whether they want to practice clinically, and identifying one core area on which to focus, as well as one to deemphasize. Trainees may also consider the possibility of additional formal training -- such as research fellowships or advanced degrees in public health, business, health administration or public policy. Noting that their proposed framework needs validation, the authors plan to monitor how it is used by both trainees and their mentors, refine their recommendations, and expand the examples of career options included in the paper.
"We need to bring as much talent as possible into this field. But until we have better defined career paths for trainees who want to engage in this work, many of them may become, at best, confused and, at worst, demoralized," Ackerly stresses. "We hope that making those career paths more explicit will lead to an energized workforce and will attract the needed institutional investments in time and resources. The struggle to improve the quality of health care should never end -- patients deserve nothing less."

We've been waiting to enjoy the promised extra detail and low power consumption of IGZO-based LCD panels for a few years now, and they're finally starting to appear. NEW YORK (AP) ? A Florida man has sued the ex-Elmo puppeteer who resigned amid a sex scandal, saying the voice actor met him in New York after trolling gay telephone chat lines seeking underage boys for sex.
The lawsuit seeking unspecified damages was filed in Manhattan federal court Tuesday by a man who remains anonymous.
The man says he met Kevin Clash on a chat line when he was 16 years old in 2000 and exploring modeling opportunities in New York.
The lawsuit says Clash had numerous Elmo dolls in his apartment when he met the teen for sex. The man, now 29, is the third to make claims against Clash, who resigned from "Sesame Street" last week after 28 years.
A spokeswoman says Clash "believes this lawsuit has no merit."

GAZA CITY, Gaza Strip (AP) ? Residents of the Gaza Strip have plastered large billboards in key locations thanking Iran for its help during a recent eight-day battle against Israel.
The posters reflect the strong ties between Iran and the Hamas and Islamic Jihad militant groups in Gaza. Israel considers Iran to be its most dangerous enemy, citing Iran's support for the militants and its suspect nuclear program.
During the fighting, Gaza militants groups fired hundreds of rockets into Israel, including Iranian-made missiles reaching as far as Tel Aviv.
The posters, displayed in three locations in Gaza City, show the Al-Aqsa Mosque in Jerusalem, surrounded by Iranian and Palestinian flags, two hands in a handshake with the words, "Thank You Iran" in Arabic, Hebrew and Farsi.
Hamas and Islamic Jihad leaders have also publicly thanked Iran.

When you think of differences between one culture to the other, the first things you think of are language, clothes, and, of course, food.
Food can be very different throughout different cultures. Sometimes we even define food by the culture it is from. ?Hey you want Chinese tonight?? or ?We should go to the Olive Garden, I?m in the mood for Italian? have become common phrases in America,? and all over the world we define food from where it comes from.
When I came to Italy, I thought there would be many exotic foods to eat. Before I came I was a vegetarian and stopped specifically because I was coming here, as I didn?t want to miss out on any dishes just because I wasn?t allowing myself to eat meat. While I have run into these situations, such as eating squid ink on pasta in Venice,? a main difference between America and Italy?s dining? experiences is when we eat what we eat.
The first meal everyone eats is of course breakfast. In America, we are constantly told that breakfast is the most important meal of the day. It should be rather hardy, because it is the start of your day. Most people have some sort of carb, such as cereal, toast or pancakes, for breakfast to give them some energy. We also will have eggs and fruit with the meal, making it a healthy but a good amount of food. Though many people don?t leave time for breakfast and will usually just grab one of these things, that is what Americans are mostly expected to eat.
In contrast, Italy tries to make their breakfast very light. Sometimes they will just drink of cup of coffee with milk sometimes with a piece of bread or pastry.
Americans eat lunch between noon and 1:00 each day. It is usually something fairly light, maybe a sandwich and a small side such as a fruit or chips.
But for Italians, lunch is normally regarded as the most important meal of the day. They usually don?t eat it until sometime after 2:00 in the afternoon. This meal usually consists of a first course, a second course and normally a dessert or fruit to go along with it.
Because Italians eat lunch a little later and have so much in that one course, they tend to not do too much snacking throughout the day. But it is normal for them to have a mid-afternoon snack sometime after 4:00. This can be true for Americans as well, though there is no specific time to snack.
Dinner in America is usually eaten between 6:00 and 8:00 and is the biggest meal of the day most of the time. Though doctors say that breakfast is the most important, dinner usually ends up in the spotlight.
There is most times a main dish of pasta or meat, a side dish of vegetables and another side dish. Sometimes Americans will have something for dessert, but most of the time this is only for special occasions.
For Italians, dinner is again a very light meal. They have lighter food such as soup, salad, cold meats, pasta, or the leftovers from lunch. Dinner is eaten sometime past 8:00.
The Italian meals seem to be much more structured than American ones, as we do not have multiple courses and snack throughout the day.
Italians also have apertivo, which is when you buy a drink and the restaurant has a light buffet spread that comes with your drink. It has been a convenient alternative to dinner when traveling, as we get a drink and a meal for only the price of a drink, instead of having to pay for a restaurant style meal every day.
So for the most part, Italians and Americans eat somewhat of the same things, just at different times. We all eat pasta, meats, and side dishes. But there are still differences I have seen between what Italians and Americans eat.
I think Italians tend eat ice cream more often than Americans. There are many more gelaterias than there are ice cream shops in the US. Of course what Italians are famous for, pasta, seems to be eaten quite frequently. Then there are the couple of things I have seen that just seem very strange, such as cow tail and rabbit, but those are probably not he most frequented dishes in Italy anyways.
There are also things we eat more in America, one being peanut butter. There are a fair amount of Italians who have never tried peanut butter! And if you?re looking for sour cream, you may have a difficult time finding it.
So if you are looking for different dining experiences then ones you are used to, but are still a little scared about completely changing your diet, an Italian one is probably pretty good to try.

Good morning all: Up here in the great Northwest, it's Fall. Today is cloudy and cool and we've had quite a bit of rain this week.
I've been meaning to write this blog for some time because I think its important. The question is how tranparent are you? The reason this came to mind is I recently had to tell a client exactly how I got paid. Granted, I've always done this but, still, most clients don't understand or pay attention.
In my case, I was working on a huge loan and had tried to submit to an East Coast bank (big mistake) who didn't understand my client's income as much of it was in the form of bonuses. After that loan submission failed,? we were scrambling as we had to pick a new lender with a higher rate and not much time to close. Needless to say, everyone was a little stressed.
I had to call my client and explain how not only was the rate higher but also how I got paid. In the first deal with the East Coast bank, I was being paid yield spread premium and wasn't charging my client any origination fee. When I submitted the loan to the second local bank, not only was the rate higher but I had to pay .25 and charge an origination fee. In the meantime, my client had gone to Lending Tree and gotten a quote for a no fee mortgage. I requested the Good Faith Estimate and saw that the competing lender had unnecessary?junk fees in the loan and a slightly higher rate.
When I called my client, I levelled with him about how I got paid. I'm not sure how happy he was about the change but hope he appreciated my candor and transparency. The bottom line is we got a loan approval yesterday and it appears we will close on time. My client will be happy as they're realizing buying their dream house. I also learned something about going with the lowest rate projected. Most of the time, the lender either won't approve the loan or they can't honor that rate. How transparent is that?
In the three and a half years I've been in this business, I've seen so much. One of the things I didn't like was working with people who were in it just for the money. They weren't transparent at all in my opinion. So that should be everyone's goal; To tell it like it is. We all have our warts. But one way to build a great business is to be as upfront and honest as we can. If not, I'm not sure we belong! Thanks for reading. WASHINGTON (Reuters) - Boeing Co said Wednesday it will restructure its defense, space and security business and cut 30 percent of management jobs from 2010 levels as part of a cost-cutting drive.
Boeing, the Pentagon's second-largest supplier, said it also will close some facilities in California and consolidate several business units in an effort to trim $1.6 billion in costs by the end of 2015, on top of $2.2 billion in reductions achieved since 2010.
"We are raising the bar higher because our market challenges and opportunities require it, and our customers' needs demand it," Dennis Muilenburg, chief executive of Boeing Defense, Space & Security, told employees in a message obtained by Reuters and confirmed by Boeing.
He said the total savings of $4 billion would make the company healthier and better able to deal with an increasingly complex and challenging marketplace.
"Even with the uncertainty ahead of us, we are charting a positive course, and we are committed to excellence, execution and investment," Muilenburg said. "I like Boeing's competitive position and our approach - facing into it aggressively and with a sense of productive urgency - and not 'hunkering down.'"
The sweeping measures come as all U.S. weapons makers are under pressure to cut costs and preserve profit margins amid dwindling defense spending in the U.S.
Boeing shares rose on news of the restructuring. Early in the day, the stock was down as much as $2.27, or 3.2 percent, at $69.31, partly on concern about dwindling defense spending following President Barack Obama's re-election. But after the news that Boeing would restructure its defense division to cut costs, the stock was trading at $70.15, still down $1.43 for the day, but off its earlier lows.
Boeing said the changes were not a direct response to the threat of additional, across-the-board budget cuts due to take effect on January 2, or the outcome of U.S. elections, but marked another step in its long-term effort to be more competitive.
Muilenburg said the company's strategy had already helped it take market share from competitors, pioneer new innovations and win orders around the world.
Muilenburg said Boeing would trim executive jobs in its defense business by an additional 10 percent by the end of 2012, eliminating many vice president and director-level jobs and resulting in a 30-percent overall cut in executive jobs over the past two years.
In addition to this "tough, but necessary work," Muilenburg said Boeing also planned to increase the ratio of non-managers to managers to a more affordable 12.5 to 1 from 9.7 to 1 now.
Together, the measures would result in a 10-percent cut in the cost of management, he said in the message to employees.
Boeing said it could not project exactly how workers would lose their jobs because it would try to place people in its growing commercial business or other areas.
A company spokesman declined to say how many jobs had already been cut from the 2010 level.
Boeing and other top weapons makers like Lockheed Martin Corp, Northrop Grumman Corp and Raytheon Co have focused heavily on cutting costs and drumming up foreign sales to maintain profits as they prepare for a sustained period of weaker defense budgets.
Lockheed reduced its management ranks by about 25 percent in recent years after announcing a voluntary buyout.
Boeing said it would also expand its efforts to cut supply-chain costs by working closely with its suppliers, but did not provide details.
Defense consultant Loren Thompson said the changes were needed to ensure Boeing's continued profitability.
"Many investors focus on Boeing's commercial operations," Thompson said, referring to the jet-making business.
"But defense provides 40 percent of the company's revenues and returns, so controlling costs there is crucial to maintaining the company's overall profitability."
Phil Finnegan, an analyst at the Virginia-based Teal Group, said Boeing and other companies were taking steps to deal with growing pension obligations and maintain profit margins that were already the lowest among the top five weapons makers.
"They're trying to stay ahead of the coming defense cuts ... and the way to do that is to cut costs," Finnegan said.
Boeing said it would merge its current mobility division with a separate surveillance business, while moving the Chinook and V-22 rotorcraft programs to a new vertical lift division.
It also planned to consolidate its missiles and unmanned airborne systems, while weapons programs such as the small diameter bomb would shift to the global strike business.
Unmanned systems would continue reporting to Chris Chadwick, president of Boeing Military Aircraft, but support functions would be shared with other divisions, Boeing said.
Boeing also planned to merge its electronic and missions systems business with a separate unit focused on information solutions to create a leaner operation.
The company said it would further cut facilities, which are already down 10 percent since 2010, including two sites in Seal Beach, California, and other locations in the state.
Boeing also announced several management moves, including the appointment of five women to new, senior positions.

WASHINGTON (AP) ? A barrage of negative ads, more than $2 billion in spending and endless campaign stops all come down to this: Americans likely will elect a Congress as divided as the one they've been ranting about for two years.
In Tuesday's voting, Republicans are poised to hold the 435-seat House, with Democrats expected to gain a small handful of seats at best from roughly 60 competitive races but fall well short of the net 25 needed for the majority. House Speaker John Boehner, R-Ohio, is poised to wield the gavel again.
Senate Democrats are likely to maintain their narrow advantage as two Republican candidates' clumsy comments about rape and abortion could cost the GOP Indiana and dampens its prospects of winning Missouri ? two major roadblocks in the Republican path to the majority.
Republicans hoped the math would work in their favor ? Democrats are defending 23 seats, the GOP 10 ? but solid Democratic recruits and the close presidential race, added to the GOP candidate stumbles may ensure that Nevada Sen. Harry Reid remains majority leader.
"That's extremely frustrating for what everyone thought was a Republican advantage," Ron Bonjean, a Republican consultant and former top Capitol Hill aide, said of the developments in Indiana and Missouri.
No matter who wins the presidency ? President Barack Obama or Republican Mitt Romney ? the nation's chief executive will be dealing with a Congress no closer to bridging the ideological chasm and showing no inclination to end the months of dysfunction. Tea party numbers are certain to tick up in the Senate with Republican Ted Cruz heavily favored in Texas and Deb Fischer looking to grab the Nebraska seat.
In the House, the movement that propelled the GOP to the majority in 2010 will be even more emboldened even if a few of the big-name tea partiers lose.
Sal Russo, head of the Tea Party Express, likened the group to the anti-Vietnam War movement of the late 1960s and early 1970s that he said remade the Democratic Party. He envisions the same with the GOP.
"In the sense that the anti-war movement brought out millions of people that had not been involved in politics and they became engaged in a material way," Russo said in an interview as he headed to what he expects will be a victory party for Cruz in Texas.
The Democratic Party, he insists, has never been the same and neither will the GOP after the influx of tea partiers.
When the Senate votes are counted, moderate Republicans and Democrats from Massachusetts and Montana could be gone, leaving the chamber with just a handful of the lawmakers inclined to reach across the aisle. Republican Sen. Olympia Snowe of Maine decided to retire earlier this year, frustrated with the partisan gridlock in Congress.
New England's three other GOP senators are New Hampshire's Kelly Ayotte, Maine's Susan Collins and Massachusetts' Scott Brown, now an underdog against Democrat Elizabeth Warren in a race for the late Sen. Edward M. Kennedy's seat.
"The few Republicans who are in office in New England are an endangered species," said veteran Democratic strategist Dan Payne, who is working for independent Angus King. "Their party has shifted so far to the right."
King is favored to win the three-way race for Snowe's seat.
A Bloomberg poll in September found that 55 percent of Americans said Congress will continue to be an impediment no matter who is elected president. Just 32 percent said Congress would get the message and work together.
Democratic strategist Steve McMahon said he worries that with a divided Congress "we can probably expect hyper partisanship and gridlock everywhere. It seems like Americans can expect more of the same."
The other certainty is neither Obama nor Romney will have much of a mandate based on the razor-thin presidential race and the likelihood that the majority party in the Senate will be nowhere near a filibuster-proof majority.
"Neither candidate will be able to claim that voters endorsed a clear and specific plan for balancing the budget because neither of them offered such a plan," said John J. Pitney, a professor of American politics at Claremont McKenna College.
Republican strategist Terry Holt said a newly elected president who has the will could put their mark on policy and make some significant changes.
"But there is so much ideological division that you will have to risk your political life to get something done in the next Congress," Holt said. "It is an all-or-nothing proposition by virtue of the divided nature of the country. You have to stick your neck out if you're to get anything done."
Weeks before the January inauguration, Congress will have to decide what to do about a $607 billion so-called fiscal cliff: the combination of expiring Bush-era tax cuts and automatic, across-the-board spending reductions to domestic and defense programs. Economists warn that no action will plunge the country into another recession.
"At the end of the day, you have so many ticking time bombs," said GOP strategist John Feehery. "Having just a complete gridlock is not an acceptable solution."
Congress may decide in the lame-duck session to delay the major decisions to early next year, especially if Romney wins the presidency. But they can't put off economic decisions for too long.
"The road to fiscal perdition is a cul-de-sac," Pitney said.
